This form is required for all new employees and MUST be submitted to the Human Resources Department in ADM 111. |

Use this form to request or change access to Banner, Discoverer, or WebXtender. |
| Blackboard Request Forms | Use this form to request staff accounts, non-Banner courses, etc. |
| Cable TV Outlet Request | Use this form to request a Cable TV outlet. |
| Computer Move Form | Use this form if you need your computer moved. |
| Discoverer Report Request Form | Use this form to request a new Oracle Discoverer report. |
| Email Account Request | Use this form to request either a departmental email account that is not tied to a specific person or an email alias. An alias is an additional email address that delivers messages to an existing account. To request login and email access for employees or temps, please fill out and submit the Temporary Login Access form (below). |

| iTunes U Course Request | Request a course space in iTunes U. |
| Laptop Reservation Request | Use this form to request a loaner laptop. |

| Network Jack Request | Use this form to request the installation of a new network jack. |
| Printing Purchase Form | Use this form to add funds to your printing account for use in the computer labs. |
| Printer Toner Request Form | Use this form to request printer toner. |
| Redirect Request Form | Use this form to request a "short" URL that will redirect to a page on the Regent website. |
| Service Request Form | Use this form to report computer problems to the Help Desk or request assistance. |
| Signature Font Request | Use this form to order a signature font. |
| Temporary Login Access | Use this form to request temporary login access for staff, faculty or non-employee. |
| Verizon Conferencing Request | Use this form to request a Verizon Instant Meeting account for telephone conferences. |
